Floyd, 39, insisted on going with her and on the auto ride over he pointed a loaded gun at her and threaten to kill her and her family, according to the report.
Floyd has been charged with domestic violence, aggravated assault with a firearm, false imprisonment, simple battery and possession of a firearm by a convicted felon.
The report said Floyd beat the woman Wednesday and refused to let her leave their home.
The woman convinced her boyfriend the dog needed to go to the vet and he agreed, however held her at gunpoint the entire time. On Friday, she convinced Floyd to take their dog to the vet.
During the spat, the couple struggled over the gun, sending two rounds through a wall inside their home.
On the drive to the vet, Floyd allegedly pointed the gun at his girlfriend, threatening to kill her and her family. The staff at the animal hospital quickly alerted Deland Police Department after receiving the note.
The note read, "Call the cops, my boyfriend is threatening me". Floyd is a convicted felon, according to the sheriff's office. She was only able to slip an employee the note once she arrived at the veterinarian's office. Please don't let him know, ' according to ABC News. Seems the boyfriend, convicted felon Jeremy Floyd, was standing beside her with a concealed and loaded weapon.
Police located two bullet holes in drywall inside the home.
The woman was taken to the hospital and treated for a head wound, a black eye, bruises on her arms and legs, and scratches on her hands and fingers, the News-Journal reported.
